This is to acknowledge that you have read and that you understand what you are committing to. The legal clauses in this disclosure are kept to a minimum. Please ensure you understand everything before signing it. This commitment agreement is not a legal document. It’s your personal commitment to me that you are serious about getting different results in your life. All the techniques and skills I use are considered either consulting or coaching or complementary health care. I am not a licensed medical doctor or Psychological practitioner. That means that I am not a Medical Doctor, psychiatrist, psychologist, Master’s in Family Therapy (MFT), or a Master’s in Social Work (MSW). The services you receive are not locally regulated. Guarantees I will provide only those services in which I have been trained, and if I find that I cannot help you, I will refer you to a licensed person who can assist you. Furthermore, nothing that happens when we meet should be construed as, nor should you believe that it is a substitute for the advice of a licensed person. To guarantee results, you must finish the program. That means, attend every session. You must go through the entire process all the way to the end. You must do the tasking after the session, and you need to do it fully and completely. My liability is limiting to the amount paid to me for the coaching sessions. Fees: All fees should be paid in cash prior to commencing the first session unless otherwise agreed. Fees are calculated as follows: INR 5000 per hour, OR INR 800 per hour for an advance payment of 6 hours. The estimated time per session is 2 hours. The Amount can be debited to my Account through bank transfer once agreement is signed between us. How do you know you have this problem? Elicit a reality strategy and any diagnoses made. 3. How long have you had it? Was there ever a time when you didn’t? What have you done about it? 4. What happened the first time you had this? What emotions were present? 5. What events have happened since? What emotions were present? 6. In each of these events, what is the relationship between the event and your current situation in life? 7. Tell me about your parents, brothers, sisters, etc. What is the relationship between these people and your current situation? 8. Tell me about your childhood in relationship to this problem. 9. Is there a purpose for this problem? Is there a reason for having the problem? Ask your unconscious mind. 10. When did you choose to have this situation created? Why? Ask your unconscious mind. 11. Is there anything your unconscious mind wants you to know, or is there anything you are not getting which, if you got it, would allow the problem to disappear? 12. Is it OK with your unconscious mind to support us in clearing this problem today, and for it to allow you to have an undeniable experience of it, when we are complete? 13. How do you know when this problem has totally disappeared? 14. So, when we get rid of ___________, will the presenting problem totally disappear? Your Life-Map As your coach, it is helpful for us both to have an understanding of the key events that have shaped your life. INSTRUCTIONS STEP 1: Note the key events that you feel have shaped your life – both negatively and positively. 1. Record your birth on the left-edge of the paper : 2. Draw a continuous line mapping your life from the past into the future, mapping the high points with peaks and the low points with valleys. 3. Write a couple of words description and your age at each of the peaks & troughs (eg. moved city, university, graduated, parents divorced etc). 4. If it helps, here are some questions you can answer along the way: What are the significant milestones/events in your life to date? What are your achievements/things you have done and are proud of? (big and small, from childhood onwards) What were you interested in as a child/what were your childhood passions? What do I need to include to make my life-map more rounded and complete? This can be a large or even a seemingly small event – it’s about the impact the event had on you and your life! NB. This exercise is meant to be completed quickly – just turn over, start drawing the line and see where it takes you!
